Update: PHCA brings Peer Mentor Program to OC

Peer Mentor Program 

Phoenix House California’s Peer Mentor Program guides participants in building a foundation to help support lifelong recovery. Our Peer Mentor team provides case management services to Drug Medi-Cal participants who are currently enrolled in Substance Use Disorder programs and require additional support to link to ancillary services. Our program will provide mentorship and help participants develop life skills to help sustain ongoing recovery.

Our passionate care coordinators are here to help guide participants toward a path of recovery and success!

SERVICES INCLUDE

  • System navigation
  • Help participants move through systems of care accessing linkage to services
  • Support with attending appointments
  • Referrals and linkage to services such as: housing, vocational and educational support, health care
    appointments and more
  • Community reintegration from structured environments
    such as incarceration, hospitalization and residential programs

We accept Medi-Cal and private insurance and provide services to those without insurance.
