Phoenix House Academy – Outpatient Center, a program of Phoenix Houses of Los Angeles, Inc., is certified by the California Department of Health Care Services, Certification No. 190115EN, for Outpatient Substance Use Disorder Treatment at 11600 Eldridge Avenue, Lake View Terrace, CA 91342. The certification is effective October 15, 2023 through October 31, 2025. Since 2003, this program has also been Drug Medi-Cal-certified for outpatient and intensive outpatient, perinatal treatment, Certification No. 1945, and certified for Medi-Cal Specialty Mental Health Services for youth by the Los Angeles County Department of Mental Health, Certification No. 7356.
